There are three main types of breast reconstruction. Autologous reconstruction using patients own tissues (such as the latissimus dorsi muscle, DIEP, or the rectus abdominis muscle, among others) , heterologous procedures (such as tissue expanders and implants) or combination of the above

Tissue expanders breast reconstruction

Following  a mastectomy, the expander may be inserted under the pectoralis major muscle or above at the same time as the procedure or at a later stage depending on the case. This way we achieve skin expansion with time ( 3 months time ) to the desired degree

The expander is, removed, and replaced by  permanent implant.  Nevertheless, there are some types of expanders that do not need to be replaced.  When the patient does not require this stage ( depends on the type of mastectomy ), the implant can be inserted at the same stage. In a subsequent stage, the nipple-areola complex may be created if necessary.

Latissimus dorsi muscle transfer for breast reconstruction

When additional skin is needed in the area to cover the defect ( depends on the type of mastectomy ) as a result of  extended excision, this approach is typically the one that is chosen as the preferred option. The latissimus dorsi muscle is moved with an island of skin through a route under the skin of the axilla to the anterior chest wall. This creates a pocket in which the prosthesis or expander can be placed. In situations when the skin of the chest wall has been treated with radiotherapy in the past, this treatment is typically the one that is recommended.

Reconstruction of the breast using a free flap (a DIEP FLAP).

In this procedure, an implant is not used at any point. It begins with the removal of a slice of skin and fat from the abdominal region, which is then relocated to the donor region (the breast), where it is kept alive through the microsurgical anastomosis of the vessels of the donor region and the recipient region under a microscope. This procedure takes between 5 and 6 hours to complete and requires a hospital stay of at least five days. The final operation to produce a nipple and areola occur six months after the initial procedure.
